ポスドク研究員の募集

我々の研究室では、H26年から開始される新学術領域研究「脳内身体表現の変容機構とその理解と制御」(代表:太田順、東京大学)のC01研究項目「脳内身体表現の変容を利用したニューロリハビリテーション」(班代表:出江紳一、東北大学)として参加することになりました。

身体所有感と免疫系

Abstract
Most people are convinced that their body parts are in fact their own, but in some clinical conditions, this sense of ownership can be lost [1]. Perceptual illusions, most famously the rubber hand illusion (RHI) [2], demonstrate that a sense of ownership over a body part (or an entire body [3]) that is not in fact ours can be easily induced in healthy volunteers. But does illusory ownership over an artificial body part have consequences for the real body part, the one that has been 'replaced'? Recent data show the RHI induces a small but robust drop in skin temperature of the real hand. That is, blood flow to the 'disowned' hand seems to be selectively reduced [1]. Such a finding is particularly relevant to the immune system because a primary role of the immune system is to 'discriminate self from non-self'[4]. We predicted that the innate immune system may be upregulated in a manner consistent with rejection of the replaced hand. Consistent with this prediction, we report here that the RHI increases histamine reactivity, which is a key final pathway of the innate immune response and has been implicated in autoimmune disorders, including multiple sclerosis [5]. Our finding has direct implications for autoimmune disorders and a range of neurological and psychiatric conditions characterised by a disrupted sense of ownership over one's body (see [1] for a list of conditions), and has broader implications that extend well beyond previous assertions about the mind-body link.

大抵の人は、自己の身体が、自分のモノであることを疑うことはない。しかし、ある臨床の場面では、このような身体の所有感が失われることがある。ゴムの手錯覚(rubber hand illusion:RHI)と呼ばれる非常に有名な知覚の錯覚では、健常者において自己の身体部位に対する所有感を、自己の身体以外の物体にも簡単に持たせることが出来る。しかし、錯覚により身体以外の人工物に対して所有感を持つことは、頭の中で実際の自分の手と置き換えられることになるのだろうか?最近のデータでは、RHIは、小さいが非常に頑健な皮膚温の低下を自己の手に生じさせることが明らかにされた。つまり、所有感を失った自己の本物の手への血流量が低下していると考えられる。この発見は、免疫系に対しても般化することが出来る。なぜなら、免疫系の第一義的な役割は、自己と非自己を区別することだからである。
そこで、この研究では、本能的な免疫系は、所有感を失った自己の手に対して何らかの免疫的な拒絶反応を示すのではないかと予測した。結果は、予測通りにRHIは、ヒスタミンの反応性上昇させた。ヒスタミンは、本能的な免疫反応の重要な最終経路であり、多発性硬化症のうような自己免疫疾患に関与するものである。本研究の結果は、自己身体への身体所有感の障害により特徴付けられる自己免疫疾患、精神・神経疾患に対する有益な情報となり、さらには、こころー身体のつながりに関しても有益な情報をもたらすだろう。

(感想)身体所有感と免疫系との関係に着眼した所がすごい。院生の会津くんが紹介してくれた論文でした。

身体イメージと身体図式の定義(Body image and body schema)

A body image is composed of a system of experiences, attitudes, and beliefs where the object of such intentional states is one’s own body.

Studies that involve body image frequently distinguish among three intentional elements.
(1)  A subject’s perceptual experience of his/her own body.
(2)  A subject’s conceptual understanding (including folk and/or scientific knowledge) of the body in general.
(3)  A subject’s emotional attitude toward his or her own body.

Conceptual and emotional aspects of body image are not doubt affected by various cultural and interpersonal factors, but in many respects their content originates in perceptual experience.

By contrast,

The concept of body schema include two aspects:
(1)  the close-to-automatic system of processes that constantly regulates posture and movement to serve intentional action,
(2)  our pre-reflective and non-objectifying body-awareness.

Body schema is a system of sensorimotor capacities and activations that function without the necessity of perceptual monitoring.
Body-schematic processes are responsible for motor control, and involve sensorimotor capacities, abilities, and habits that enable movement and the maintenance of posture.
Such processes are not perceptions, belief, or, feelings, but sensorimotor functions that continue to operate, and in many respects operate best, when the intentional object of perception is something other than one’s own body.
On the other hand, however, the body schema (and this reflects Merleau-Ponty’s usage of the term) also includes our pre-reflective, proprioceptive awareness of our bodily action.

小脳と運動観察

我々は経験的に、少脳皮質損傷より大脳皮質損傷のほうが認知機能への影響は大きく、そして重要と考える。しかしながら、神経イメージング手法を用いた研究では、運動観察や運動理解に小脳が非常に強く賦活することが一貫して報告されている。このことは、小脳が運動観察において生じる自動的シミュレーション処理の一部に密接に関与しているからと考えられている。運動実行時に小脳がシーケンサー(sequencer)のように機能することより、小脳が運動を構成する要素の時間的処理(タイミング)を理解するのに非常に重要な役割を担っていると考えられる。そこで、慢性期で初発の小脳の虚血性血管障害8人でコホート研究を行った。実験課題は、他者による複雑な運動、または物体の複雑な運動という2種類の運動からランダムに埋め込まれている異質な運動シーケンスを検出するという課題である。16名の健常被験者が統制群として参加した。結果、小脳損傷の患者では、統制群と比較して、両運動条件において有意に成績の低下が見られ、さらに、ヒトの運動条件での成績が物体の運動条件に比較し成績が低かった。このような乖離は、ヒトの運動シークエンスは、他のシークエンスとは異なって小脳で表象されているようである。


(感想)
今水さんの左小脳皮質の内部モデルの話からかもしれないのですが、どうも小脳損傷患者さんには我々が行うよ観察・模倣運動学習の効果が難しいような気がします。一度、機会があれば障害の小脳半球の左右差を観察・運動学習で見てみたいと考えてます。

tremor (振戦)

振戦(tremor)とは、相反神経支配されている筋肉群(主動筋と拮抗筋)が交互または不規則に収縮することにより引き起こされる不随意でリズミックな運動である。運動がリズミックであることにより他の不随意運動と区別され、主動筋と拮抗筋が関与することによりクローヌス(clonus)と区別される。

異常な病理的な振戦は、臨床上四肢の遠位筋でよく見られ、頭部、舌、顎、体幹筋といった近位部の筋は非常に稀である。また振戦は、覚醒下においてのみ見られ、殆どの場合、4から7Hzという生理的振戦の約半分ほどの周期である。

臨床の解析では、振戦の1)リズム性、2)周波数、3)運動や姿勢との関係、4)拮抗筋の活動パターン(交互または不規則)により分類される。

痛みと身体図式

痛みと身体図式、この2つの関係は幻肢痛へのミラーセラピーの効果を考えると結びつくものだけど、具体的にどのように結びついているかはわからない。

幻肢痛では、ない四肢があると感じるという幻肢という状態が身体図式の異常であることはまちがいない。また、ミラーセラピーでは、幻肢の運動意図と視覚フィードバックを一致させることにより、身体図式の修正、または再構築が行われている可能性が高い。

だけど、なぜ身体図式の異常が痛みにつながるかはわからない。幾つか仮説はたつけど、なかなか検証する方法が難しい。

ちなみに週1で、幻肢痛ではないがCRPSのtypeI疑いの患者さんへのミラーセラピーの介入を行っている。正直、効果は微妙であるが、長期的(半年くらい)にみると痛みは減少している。ただ、週一の介入で半年も続けてると、一体何が効いているかを知るのは困難だけど。

まあ、研究的介入であるのでじっくりと時間をかけて患者さんをみて、なにかヒントを得られればとのんびり構えています(^^


研究キーワード

研究の興味は、
「身体図式とは一体何なのか?それはどのように構築、再構築できるのか?」

現在の研究プロジェクトは、
1,慢性期脳卒中片麻痺患者への経頭蓋磁気刺激と模倣運動学習
2,幻肢痛に対する身体図式の再構築
3,小児に対する経頭蓋電気刺激(tDCS)を用いたリハビリテーション法の開発
でやってます。

研究のスタイルとしては、大規模研究ではなく、協力していただいている患者さんをできる限り長期にわたり細かくしっかりと見ていく狭く深くのスタンスです。
なもので、症例数は遅々として増えません(^^;
それで、今流行のRCTなんぞ先の先の話です。
このようなスタンスをとっている理由は、学術的なものではなく私の性格に起因しているような。。。

3の小児の研究は、生まれながらにして持っている模倣運動を利用した模倣運動学習の研究をしているので、子供のリハになんとか応用できないかというのがきっかけです。まだ始まったばっかりで現在6歳の子を週二回、半年にわたり見て、上肢の不随意運動の抑制を目指して、ほそぼそとやっています。
